We’ve seen two failures, one intermittent and one definitive:

Intermittent due to race condition: Bug #28851: Race condition when creating roles via plugin interface - Foreman
The definitive issue has been fixed and nightlies are all green. The next big changes we’ll be looking to merge in order are:
- Postgres 12 upgrade for Foreman and Katello scenarios: https://github.com/theforeman/foreman-installer/pull/444
- Enabling Pulp 3 by default
